Lost ID Card Replacement Request Form -- For Lost IDs Only!

Only use this form if you have LOST your current Rensselaer ID card and would like to request that a new one be issued.  Do NOT use this form if you have a broken or worn ID card.  If you have a broken or worn ID card and would like to trade it in for a new one, please use the Broken or Worn ID Card Replacement Request Form.

Requests are usually processed within two business days of the initial request during the hours of 8:30am to 4:00pm Monday through Friday, excluding holidays.  The Campus Card Office will contact you at the email address entered below to provide you with a locker location and combination, as well as the pickup dates and times you can collect your new ID card.

If you are a student or employee, the Campus Card Office will contact you at the email address entered below to provide you with a locker location and combination, as well as the pickup dates and times you can collect your ID card.  You will need to return the broken or worn ID card at the time of pickup, or you will be charged a fee based on the lost ID card fees shown below. 

If you are not a student or employee your card will interoffice mailed to your sponsor/representative.

STUDENTS: Please note that once you click Submit below, your student Bursar account will be immediately charged the appropriate replacement fee of either $10 for a first card loss, or $25 for a second card loss or more.  The ID card replacement fee is non-refundable.

If you have been given a temporary ID card from Public Safety, you will have to return that card at the time of pickup as well, or you will be charged $25 to reprint that temporary ID card. The temporary ID card replacement fee is non-refundable.
